All organizations listed below are conditionally-approved ACO Realizing Equity, Access and Community Health Consumer Assessment of Healthcare Providers and Systems (ACO REACH CAHPS) Survey vendors. They have met the requirements for conducting the survey on behalf of REACH ACOs and have successfully completed CMS’ training and certification on the ACO REACH CAHPS procedures. Survey vendors with an asterisk (*) next to their name conduct both the Standard/New Entrant and High Needs surveys.

Please note this is not the final list of approved vendors for 2024. The final list of approved vendors will be available in June 2024. REACH ACOs should wait until June 2024, when the list is final, before completing the Vendor Authorization process on this website.
