2025 ACO REACH CAHPS Survey Vendor Application Period Closes Soon

The Performance Year (PY) 2025 survey vendor application period for ACO REACH CAHPS will close on January 21, 2025.

To apply to become an ACO REACH CAHPS Survey vendor, organizations must:

• Meet all the Minimum Survey Vendor Business Requirements at the time of application submission, including being approved for mixed mode (mail with telephone follow-up) by another CMS CAHPS survey.
  • Vendors who are currently operating remotely for any portion of their mail and/or telephone operations and wish to continue remote operations for ACO REACH CAHPS must submit an Exceptions Request Form (ERF) to CMMI for review for approval.
  • The online ERF form is available for organizations that have registered for vendor credentials. Submit the ERF after submitting the vendor application. The ERF is located under the "Vendors" tab once vendor applicants have logged into the ACO REACH CAHPS website.
• Previously approved ACO REACH CAHPS survey vendors need only update their approved remote operations ERF if they have made changes to their remote operating protocols since it was last approved.
• Complete and submit the application form by January 21, 2025.

REACH ACOs will receive a list of conditionally approved ACO REACH CAHPS survey vendors in April 2025 and fully approved ACO REACH CAHPS survey vendors in June 2025.
