Survey Vendor Application Process

Only invited survey vendors can apply to become an ACO REACH CAHPS Survey Vendor. Invited survey vendors interested in becoming a CMS-approved ACO REACH CAHPS vendor will need to meet a number of business requirements described in the ACO REACH CAHPS Minimum Business Requirements available here.

The application and approval processes will consist of the following:

  • Completing and submitting Vendor Registration and a Vendor Application and demonstrating experience conducting mixed-mode surveys (mail and telephone). If you are interested in becoming approved to administer the ACO REACH CAHPS, please contact the ACO REACH CAHPS Survey Coordination Team via email for more information at acoreachcahps@rti.org.
  • Participating in the ACO REACH CAHPS Survey Vendor Training session and successfully completing the training certification. The next ACO REACH CAHPS Survey Vendor Training sessions will be held in early April 2024. The training session will be self-paced for new vendors, and a live webinar for returning vendors.
  • Completing and submitting a Quality Assurance Plan. Survey vendors will be required to complete and submit a Quality Assurance Plan after attending the ACO REACH CAHPS vendor training in order to be approved for vendor authorization.
