Welcome to the Accountable Care Organization Realizing Equity, Access, and Community Health (ACO REACH) CAHPS, (formerly GPDC CAHPS) survey vendor registration process.

Only vendors invited by email invitation can apply to be a vendor on the ACO REACH CAHPS. If you have questions about your eligibility to apply to be an ACO REACH CAHPS vendor, please contact us at acoreachcahps@rti.org or 1-833-870-0486.

Each invited survey vendor interested in submitting an application to become an approved ACO REACH CAHPS vendor must designate a staff member to be its ACO REACH CAHPS Survey Administrator/Project Manager, who must then register by completing the information below. Once your Survey Administrator submits this form, they will be directed to the Vendor Dashboard, where the application is located.

Organizations applying to become a ACO REACH CAHPS vendor are required to create an account to access specific links and online forms on the ACO REACH CAHPS website. Please create and enter below a Survey Administrator Username and password you will use to access the private links and forms on this website.
